DIY CV Offset/Scaler module based on a quad OpAmp

Can offset and attenuate/amplify (up to supply voltage less ~1.5V).
E.g. 0-5V from SQ-1 sequencer can 'play' the Phonogene's 'Slide' CV in which requires ~+/-8V for full range.
Bipolar LEDs to indicate relative input and output voltage level.
Also includes an inverted output.
With no input, generates a fixed voltage.
